Why is diaphragm pain felt in the shoulder?

The phrenic nerve carries sensory fibres from the central diaphragm and arises from spinal segments C3–C5. Those same segments supply the skin over the shoulder tip through the supraclavicular nerves, so the brain reads the signal as coming from the shoulder.
